After completing the online registration process you will receive a confirmation email with your unique confirmation number.

**Note:** You will not receive tickets in the mail. Your name will be at Will-Call at our Registration desk. Keep your confirmation email as your receipt.

Mail in Registration

1.	Click to download the Registration Form
2.	Fill out form completely and print
3.	Attach check payable to: LSDC
4.	Mail completed form and check to:

	Liberty Swing Dance Championships 211 E. Hobart Gap Road Livingston, NJ 07039

You will need to fill out the Formation Team Waiver Form if you are registering for the Formation Team Division. This form needs to be filled out and mailed to LSDC.
